About this study
This is a short questionnaire; I would be extremely grateful if you could spend a minute or two completing it. There are just a few points that you should be aware of before you start: This questionnaire deals with issues related to fertility, conception and sexuality. If you feel uncomfortable with any of these issues, now or at any point in the study, you are not obliged in any way to complete the questionnaire. You are assured complete anonymity; at no point will you be asked to state your name.
