About this study

Stanford has received a grant to study the quality of data collected on the Internet. To do that, they need people with ANY chronic disease to fill out a questionnaire just once. Participants will be randomized to either fill out the questionnaire on the Internet or by US Mail, and will receive a $10 Amazon.com gift certificate in appreciation. This study will help Stanford learn more about conducting research on-line for people with chronic health conditions.
